Purpose: We have built a draft of the school website. Many sections currently use
placeholder text or example photos so the design could be reviewed early. To finish the real website,
we need the school to fill in this form with accurate, final information — and to hand over the
documents/photos listed in each section.
How to fill this in: Write directly in the blank spaces/boxes. If a question does
not apply to the school, write "N/A" rather than leaving it blank, so we know it wasn't missed.
Each question has a short note in italics explaining what it means and, where useful, what
the website currently shows as a placeholder.
Photos and documents: Please do not print and hand-write descriptions of photos —
instead provide the actual photo files (on a pen drive, WhatsApp, or email) and just write the file
name or a short label in the box, so we know which photo belongs where.
Confidentiality and use of information. This form is exchanged between
Jnana Degula Academy / DCS Education Trust ("the School") and
Staviontech Pvt. Ltd. and Krishnabhirama Marketing Pvt. Ltd.
("the Service Provider") solely for the purpose of collecting and confirming
content for the School's public website. The School agrees to keep this document and its contents
confidential, and not to share, forward, copy, or disclose it — in whole or in part — to any third
party, without the Service Provider's prior written consent. Any personal information about staff,
students, or families disclosed in this form (including names, photographs, and contact details)
will be used by the Service Provider only for building and maintaining the website described above, and
will not be shared with any third party except where necessary to publish it on the website at the
School's direction. By signing the acknowledgment at the end of this form, the School confirms that
it has the right to share the information and photographs provided, and authorizes the Service Provider to
publish them on the website. This document does not itself constitute the commercial agreement
between the parties; it is a content-collection and content-approval record made under that
agreement.
